Immunization Waiver Information
As of January 2015, Michigan school districts are no longer permitted to distribute immunization waiver forms and can only accept the forms from the County Health Department. Parents are encouraged to contact their child's medical provider to arrange for any needed immunizations or their County Health Department of residence to arrange for an appointment for the educations session required to obtain the waiver.
